4. Is Long-Term L-Carnitine Use Proven Safe?

Not in the sense most people mean by that question. L-carnitine has been studied for months and, in some medical populations, longer. But long-term studies are heterogeneous, often use higher doses than Biogénique, and are not designed to prove that indefinite supplementation is risk-free for a healthy person.

NIH notes that carnitine has no established tolerable upper intake level, but doses around 3 g/day can cause gastrointestinal symptoms and fishy body odour. NIH also describes an unresolved research question around gut-microbiome conversion of carnitine to TMAO and possible cardiovascular implications. The fact that the implications remain uncertain is a reason to avoid both alarmism and complacency.

Biogénique’s labeled daily L-carnitine exposure is lower — 500 mg to 1,000 mg/day — but dose comparisons do not erase the separate green-tea extract safety issue. A combination product should be judged as a combination product.

The current Health Canada licence for NPN 80086440 explicitly states that rare, unpredictable cases of liver injury associated with green tea extract-containing products have been reported in Canada and internationally. NCCIH similarly reports that liver injury is uncommon but has occurred primarily with green tea extracts in tablets or capsules. The correct interpretation is neither “green tea extract is dangerous” nor “rare means ignore it.” It means know the warning signs before you start.
