Iron With Vitamin C: Does It Actually Improve Absorption?

A practical guide to 40 mg iron bisglycinate, vitamin C, fatigue, absorption and safety

Iron supplements get complicated surprisingly fast. One bottle says ferrous sulfate. Another says bisglycinate. Some add vitamin C. Some promise “maximum absorption.” Then there is the dose: 18 mg, 25 mg, 40 mg, 65 mg. If you are simply trying to make a sensible choice, it can feel like a lot of chemistry for one mineral.

The useful question is not whether vitamin C and iron belong in the same conversation. They do. Vitamin C can improve the absorption of nonheme iron. The better question is how much that matters in a real oral-iron routine—and whether a 40 mg supplement is actually appropriate for you.

QUICK ANSWER

Vitamin C can enhance nonheme iron absorption, but clinical trials in people being treated for iron-deficiency anemia have not shown that adding extra vitamin C always produces a meaningful improvement over oral iron alone. Why vitamin C gets paired with iron

Iron from food comes in heme and nonheme forms. Heme iron, found in animal foods, is generally more bioavailable. Nonheme iron is found in plant foods and is more sensitive to the rest of the meal. The NIH Office of Dietary Supplements notes that ascorbic acid—vitamin C—enhances the bioavailability of nonheme iron. That is the physiological basis for pairing iron-rich foods with vitamin-C-rich foods and for adding vitamin C to some iron supplements. [4,5]

That mechanism is real, but it is easy for supplement marketing to stretch “can enhance absorption” into “you need vitamin C or your iron will not work.” The clinical evidence is more nuanced.

Does extra vitamin C make oral iron work better?

A 2020 randomized clinical trial enrolled 440 adults with newly diagnosed iron-deficiency anemia. Participants received oral iron plus vitamin C or the same oral iron without vitamin C for three months. The two strategies were equivalent for early hemoglobin recovery, and the ferritin change at eight weeks was not significantly different. [7]

A 2024 systematic review and meta-analysis looked at 10 randomized trials plus one prospective cohort, representing 1,930 patients. Adding vitamin C produced a statistically significant but small increase in hemoglobin—about 0.14 g/dL—and the authors judged the effect likely clinically insignificant. Their conclusion did not support routine vitamin C supplementation with oral iron for the treatment of iron-deficiency anemia. [8]

So what should a consumer take from that? Vitamin C has a legitimate role in iron physiology, and putting the two ingredients together is convenient. But “vitamin C is mandatory” or “this guarantees maximum iron absorption” would oversell the evidence. Biogénique’s current Health Canada licence for this product does not make a separate iron-absorption claim for the vitamin C. [1]

What does 40 mg of iron bisglycinate actually mean?

The 40 mg on the Biogénique label is the amount of elemental iron supplied from iron(II) bisglycinate—not 40 mg of the full chelate compound. Each capsule also contains 100 mg of vitamin C as ascorbic acid. [1,2]

For context, Health Canada’s dietary reference values list an iron RDA of 8 mg/day for adult men aged 19–50, 18 mg/day for adult women aged 19–50, and 27 mg/day during pregnancy. The adult tolerable upper intake level is 45 mg/day. [3]

Those numbers do not mean a licensed 40 mg supplement is “too high.” The UL is designed for apparently healthy populations and is not a treatment ceiling; Health Canada specifically notes that medically supervised treatment can involve intakes above a UL. But 40 mg is clearly a targeted iron dose, not a casual “just in case” multivitamin amount. That matters if you already take a prenatal, multivitamin or another iron product. [3]

DOSE REALITY CHECK

One Biogénique capsule provides 40 mg of iron. Because the adult UL is 45 mg/day for normal chronic intake, do not casually stack this with other iron-containing supplements. The right dose depends on why you are taking iron, your diet, life stage, bloodwork and clinical context.


Is iron bisglycinate really “gentler”?

Ferrous bisglycinate is often marketed as a better-tolerated form of iron. There is evidence behind that idea, but it is not universal. A 2023 systematic review and meta-analysis included 17 randomized trials. In pregnant women, 4–20 weeks of ferrous bisglycinate produced higher hemoglobin than comparator iron supplements and fewer reported gastrointestinal adverse events. The pooled incidence-rate ratio for GI events was 0.36. However, the evidence was strongest in pregnancy, results in children were not significantly different, and the authors said more trials are needed in other populations. [6]

That is a good example of why product-specific wording matters. The study supports the form as a reasonable option; it does not prove every person will tolerate every bisglycinate product better. Biogénique’s own licensed label still warns that some people may experience constipation, diarrhea and/or vomiting, and advises stopping use if hypersensitivity occurs. [1,2]

Who should consider a 40 mg iron supplement?

Iron is essential for hemoglobin, the red-blood-cell protein that carries oxygen from the lungs to tissues. Iron deficiency can progress from depleted iron stores to iron-deficiency anemia, and fatigue, weakness and difficulty concentrating can occur when anemia develops. [4]

But fatigue is not a diagnosis. Sleep loss, thyroid problems, infection, low B12 or folate, blood loss and many other issues can feel similar. If you have persistent fatigue, shortness of breath, palpitations, unusual weakness, heavy menstrual bleeding, or an abnormal blood test, the goal should be to identify the cause—not simply pick the strongest iron bottle.

A higher-dose iron supplement makes the most sense when there is a clear reason for iron supplementation: a clinician has recommended it, your intake or life stage creates a meaningful gap, or bloodwork has established low iron status and the underlying cause is being addressed. People who menstruate and people who are pregnant have higher iron requirements on average, but that does not make the same 40 mg dose automatically appropriate for everyone. [3,4]

How to take iron without creating avoidable problems

The product label is the first instruction to follow: adults take one capsule once daily with food, a few hours before or after other medications or natural health products. [1,2]

That spacing matters. Iron can reduce the absorption of levothyroxine and levodopa, and proton-pump inhibitors can reduce iron absorption in some people. Calcium can also interfere with iron absorption, which is why experts commonly recommend taking separate calcium and iron supplements at different times. [4]

Food timing can be a trade-off. Taking iron with food may be easier on the stomach, while some meal components can reduce absorption. For this product, follow the licensed direction to take it with food rather than inventing a fasting protocol from another study or another iron formulation.

Should I always take vitamin C with iron?

No. Vitamin C can improve nonheme iron absorption, but randomized trials in iron-deficiency anemia show that extra vitamin C is not always necessary for a meaningful treatment response. A combined product can be convenient, but “mandatory” is too strong. [5,7,8]

Is 40 mg of iron a lot?

Is iron bisglycinate better than ferrous sulfate?

Some trials—especially in pregnant women—suggest ferrous bisglycinate can be effective with fewer gastrointestinal adverse events than comparator iron supplements. The evidence is not strong enough to declare it universally superior for every population or every person. [6]

Can I take this with my thyroid medication?

Iron can reduce levothyroxine absorption. The NIH notes that some levothyroxine labels advise keeping iron supplements at least four hours away. Biogénique’s label more generally says to take this product a few hours before or after other medications or natural health products. Confirm your timing with a pharmacist or prescriber. [1,4]

The bottom line

Iron and vitamin C make biological sense together, but the best evidence does not support turning that fact into a dramatic absorption promise. Vitamin C helps nonheme iron absorption; in clinical treatment studies, adding vitamin C to oral iron often produces little or no clinically meaningful advantage over iron alone.
