Does Creatine Cause Hair Loss? What a 2025 Randomized Trial Actually Found

A supplement can be effective and still lose the sale if the customer is afraid of what it might do to their hair.


The hair-loss fear did not come from nowhere - but it grew far beyond the evidence

Creatine and hair loss became linked largely because of concern around dihydrotestosterone, or DHT, a hormone involved in androgenetic hair loss in genetically susceptible people. A small 2009 study in male rugby players reported an increase in DHT after a creatine protocol. It did not measure hair loss.

Over time, the internet simplified that chain of events into a much stronger statement: creatine causes baldness. That statement was never directly demonstrated.

In 2025, researchers finally measured hair outcomes directly

A 12-week randomized, double-blind, placebo-controlled trial enrolled resistance-trained men aged 18-40. Participants took either 5 g per day of creatine monohydrate or placebo. Researchers measured testosterone, DHT and several direct hair outcomes, including hair density, follicular unit count and cumulative hair thickness.

Thirty-eight participants completed the study. The researchers found no significant differences between creatine and placebo for DHT, testosterone-related comparisons or the measured hair outcomes.

What the trial means

This study provides the strongest direct evidence to date against the claim that 5 g per day of creatine monohydrate causes measurable hair loss over 12 weeks in healthy, resistance-trained young men.

That is reassuring. It is also not the same as proving that creatine could never affect hair in any person under any dose, duration or genetic context. The trial was limited to men, lasted 12 weeks and did not determine family history of hair loss. Responsible marketing keeps those limitations visible.

Why this is a trust opportunity

Fear-based objections are where supplement brands often overreact. One side says 'creatine definitely causes hair loss.' The other side says 'that is completely impossible.' Both positions are more confident than the evidence allows.

A better brand says: here is where the concern came from, here is the newer direct trial, here is what it found, and here is what remains unknown. That is how authority is built.

Should you stop creatine if you notice hair changes?

Hair shedding can have many causes, including genetics, illness, stress, nutrition, medications and dermatologic conditions.
